Registration
The registration fee for the meeting includes educational materials,
breakfast, and lunch.

Cancellation requests must be received by October 11, 2008. No refunds
will be made after that date.

The Conference Center at NorthPointe at 866.233.9393 for room reservations
by September 12, and mention the Ohio-ACC meeting. Room reservation rates
INCLUDE dinner on Friday, October 17, in the dining room from 5:30-8:00 pm
(or boxed dinner for late arrival with prior notification), and overnight
accommodations.
The
special discounted rates are as follows:

Meeting attendee single occupancy is $195 + tax
* Meeting attendee double occupancy is $145 + tax per meeting attendee

Additional charge for guest sharing room who is not attending the meeting:
$65 + tax (includes dinner on Friday, October 17 + lunch in the dining
room on Saturday, October 18)
